First I tought it was the game, then the Mouse

I created a profile for Notepad with the keyboard key (L) mapped to "S" and got the same problem.

Occasional Double input on Single Press show up for me if a Double Press is also mapped on the same button, with something else like "T" for example.

Press Times at default values in the settings.

I'm afraid I don't understand the "Occasional Double Input" part.

There seems to be an issue with S being produced only 3 times out of 4, so we will look into it. But I couldn't get any double inputs with this config.

Having both Single and Double Press activators on the same key makes the Single activator trigger with a delay.
Maybe you should consider using a modifier key (Shift Jump) for the alternative mapping?

May be it's related to your issue but in my case I sometime get a double S while pressing L one time. (quick press, so no times for activating keyboard auto repeat). Sometime it can work fine for like 40 presses then appear every 10 press or so. Anyway if can't find the issue I will try to re-install the app and see.Thanks.

This could happen if you set the Press time (Preferences) for Long/Double/Triple too low.
